What services are available in a SNF? What does Medicare cover? LifePlans’ Provider Pathway works with you to locate the appropriate facility, obtain the contracted discount and help facilitate the admissions process.
Skilled nursing facilities also known as nursing homes, provide care and supervision 24 hours a day, 7 days a week. People recovering from an acute episode of illness such as a stroke or hip fracture may be transferred from the hospital to a Rehabilitation Unit in a SNF for a relatively short stay often covered primarily by Medicare. |

SNF’s also provide custodial care for those people who are no longer able to care for themselves and need assistance with basic activities such as bathing, dressing and eating. SNF’s also provide a safer and more secure environment for people with dementia who may wander if left alone. The number of people with dementia is increasing dramatically and many skilled nursing facilities have developed specialized units designed specifically to provide a safe and secure environment for people with dementia. If the senior you care about has Alzheimer’s disease or another form of dementia, a skilled nursing facility with a specialized Dementia Unit may be a valuable resource for you.

Hal contacted the Provider Pathway at the suggestion of his elderly parents' long-term care insurance company. Both of his parents are now over 90 and need to relocate to a skilled nursing facility after having a 24/7 home health aide for several months.
